I confirm that the details provided by me are true and accurate. I give my consent to ASRIJ Foundation to share my blood group and contact details with the patient/family who has raised the blood request, to help facilitate the donation.

I understand that donating blood is entirely voluntary on my part, and that ASRIJ Foundation is a facilitator only — it does not provide medical services and is not liable for any outcome related to this donation.

I confirm that this is a voluntary, non-commercial donation. ASRIJ Foundation will not pay me any amount for this donation, and I will not charge or claim any amount from the patient/family in exchange for my blood. This donation is made purely as a charitable contribution on my part.

I understand that ASRIJ Foundation is a facilitator only. It does not provide medical services, does not guarantee donor availability, and is not liable for any outcome related to donor matching or blood donation.

I confirm that I am joining ASRIJ Foundation as a volunteer purely on a voluntary, unpaid basis. I will not take, demand, or claim any amount or favour from any patient or their family in exchange for my services as a volunteer.

I understand that if I take or demand any charges from a patient or their family at any point, it will be entirely my own personal responsibility, and ASRIJ Foundation shall not be liable for such action. ASRIJ Foundation reserves the right to take appropriate action against me, including but not limited to removal from the platform/organization, in such a case.
